For support coordinators

For support coordinators and referrers

Referring someone to Green Pathways

A quick guide to how we work, who we support and what we need from you.
How referring works

Four simple steps

1. Send a referral

2. We reply

We get back to you within one business day.

3. We talk about goals

We arrange a welcome meeting with the participant and family, and with you if you would like to be there.

4. Agreement and start

We go through the service agreement and pricing, then set a schedule together.
Who and what we support

What to expect

Funding we work with

Self-managed, plan-managed and privately funded participants. We cannot currently support plans managed by the NDIA.

Where we support

Geelong, the Bellarine, the Surf Coast and South Gippsland, from teams based in North Geelong and Leongatha.
Good to know

Before you refer

Complex needs and behaviour support

Experienced supporting people with behaviours of concern and following Behaviour Support Plans. Our Registered Nurse helps guide how we support people's health needs.

High intensity supports

We are working toward registration for high intensity supports. If someone needs them, book a call back and we will be upfront about what we can support right now.

Pricing

Aligned with the NDIS Pricing Arrangements, at the same rates for everyone.
Helpful to include

What we need in a referral

  • Who the referral is for, and the best way to contact them
  • Which support they are after
  • How their plan is managed
  • Anything that helps us match the right team

Not sure? Just book a call back and we will talk it through.
